What do emergency doctors do when a person is having a seizure?
Not very much, usually. Most are short and self-limited, requiring no immediate treatment. And the most common seizure seen in the emergency department is in a patient who hasn’t been taking enough of his medication for chronic seizures. He’s likely to have been too busy to refill his medication and just needs to be taking it. This tends to be distressing to friends and family members who aren’t used to it, because it seems the physician isn’t “doing enough,” when there’s really not much to be done.
Of course, there are other reasons for people to have seizures. Perhaps it’s a diabetic who’s skipped a meal and is now hypoglycemic. Perhaps it’s an alcoholic who’s been cut off and is in alcohol withdrawal. Once every few years I’ll pick up a brain tumor or stroke as the cause. Usually a decent history and exam will tease these out without much trouble.
The seizures that progress to status epilepticus are a different matter, but that should be distictly delineated from the run-of-the-mill seizure.
